The Rise of Reciprocal IVF: How Both Partners Can Share the Journey

For same-sex female couples, one of the most meaningful and increasingly popular paths to parenthood is reciprocal IVF — sometimes called shared motherhood, co-IVF, or partner IVF. What Is Reciprocal IVF? 

Reciprocal IVF is a form of in vitro fertilization designed specifically for same-sex female couples. In this process: 

  • Partner A (the "genetic mother") undergoes ovarian stimulation and egg retrieval. Her eggs are fertilized in the laboratory using donor sperm. 

  • The resulting embryo is transferred into Partner B (the "gestational mother"), who carries the pregnancy and gives birth. 

The result is a baby who is genetically connected to one partner and carried by the other — a beautiful way for both women to share in the biological story of their child. 

How Does the Process Work, Step by Step? 

While every patient's experience is individualized, here is a general overview of how reciprocal IVF typically unfolds: 

Step 1: Initial Consultation and Evaluation 

Both partners will come in for a thorough consultation and fertility evaluation. This includes bloodwork, hormone testing, and ultrasounds for the partner providing the eggs, as well as a uterine evaluation for the partner who will carry the pregnancy. This helps our team design a protocol that gives both partners the best chance of success. 

Step 2: Ovarian Stimulation and Egg Retrieval 

The partner providing the eggs will undergo ovarian stimulation using hormone medications, followed by a minimally invasive egg retrieval procedure. This is a standard step in IVF and typically takes about 10 to 14 days of monitoring before the retrieval itself. 

Step 3: Fertilization and Embryo Development 

The retrieved eggs are fertilized in our state-of-the-art embryology lab using donor sperm. The resulting embryos are monitored over several days as they develop. Our team may recommend preimplantation genetic testing (PGT) to evaluate embryo health and improve success rates. 

Step 4: Uterine Preparation and Embryo Transfer 

While the embryos are developing, the carrying partner begins a protocol to prepare her uterine lining for implantation. When the timing is optimal, one embryo is transferred into her uterus in a brief, minimally invasive procedure. 

Step 5: The Two-Week Wait and Pregnancy Test 

After the transfer, both partners enter the two-week wait before a pregnancy test confirms whether implantation was successful. This is often the most emotionally intense part of the process — and our team is here to support you through it.
