Only New Patients to the practice are eligible and must register online. The entry form must be completed in its entirety; you must be at least 21 years old and seeking to start a family. Only one entry per person, a consultation with one of our physicians must be scheduled to complete eligibility requirements. The consultation is to determine if this procedure is medically necessary. Once it is determined that IVF will be your best option to conceive, your entry will be completed. A winner will be announced on July 1^st and January 2^nd of each year commencing July 1^st 2009 and will continue until the raffle is no longer offered. We reserve the right to discontinue the raffle at any time. This offer is non-transferable. Only the winning couple can obtain the free services. The raffle is for a basic IVF cycle. This offer cannot be combined with any other procedure or discounts. This offer does not have any monetary value and no monies will be paid out to the winning couple. This offer is for services only, no preliminary test, procedures, surgery or medication is included in this offer. If for any reason you decide to have the procedure prior to the announcement date your name will remain in the raffle and you will obtain a refund in the amount covered by the raffle for services you originally paid for. This will only occur if all eligibility requirements were met during the raffle period. Registrations for the raffle period will end 45 days prior to the announcement date. Anyone registering after that will be eligible for the next raffle cycle.  Good luck!
